Immigrant Visa Forms
Persons with petitions which were approved before the current priority date for their visa category should have received an instruction package from the National Visa Center.
The package sent by the National Visa Center includes a number of forms, two of which must be returned the Embassy before an appointment can be scheduled (forms DS-230 part 1).
These forms should only be sent to us when you have collected all the required documents and are ready for your interview. You may mail or fax these forms to us. Click here to see our contact information.
Also, do not schedule your medical examination until you have been told that your case is at the embassy and ready for processing. Please remember that all medical examinations must be conducted by our panel physicians. See the packets below for information on how to contact a panel physician.

Employment based immigrants and applicants for fiancé visas should prepare the I-134 affidavit of support form (pdf 69 kb) in lieu of the I-864 series.
Note: Do not submit these forms unless you have been notified by the national visa center or the Embassy that your case is ready for processing and you have all your documents ready to submit.
You may also call the regional information center to see if an appointment has been made, or to change an existing appointment. You will need to bring all your original and signed documents plus both parts of form DS-230 to the interview. Each applicant must complete all forms. Do not send the forms to us before your interview.
